Mesothelioma A.K.A. Asbestos Cancer

Mesothelioma is a painful and deadly cancer that occurs in the mesothelium or the thin layers of tissue that serve as a lining for the internal organs.  It is most commonly found in the pleura cellls in the lungs, but does occasionally occur in the heart or abdominal cavity.
It is commonly called asbestos cancer as the primary known cause of this malignancy is exposure to this single hazardous material, either through inhalation or ingestion.  Asbestos was widely used in the twentieth century in industrial applications. It was commonly found in plaster, drywall, insulation, flooring, roofing, ceiling tiles and automotive brakes, as well as being used in various fireproof materials.
Exposure was common among blue collar workers in particular, although asbestos fibers also found there way off of job sites and into the home on workers’ clothing and shoes. A large proportion of mesothelioma cases occur among former employees of constuction, railroad, shipping, manufacturing and mining trades.
A diagnosis of mesothelioma may come as a surprise because it generally does not occur until long after exposure to asbestos, commonly between ten and forty years later.  Symptoms may be vague and go ignored for months or be mistaken for more common ailments such as pneumonia or influenza. Signs commonly include shortness of breath and coughing (when it occurs in the lungs), fatigue, low back or side chest pain, weight loss, anemia and sometimes hoarsene Due to the fact that the chances of successful treatment increase dramatically if mesothelioma is caught in the earliest stages, it is vitally important that anyone who develops these symptoms and has reason to suspect prior exposure to asbestos seek medical advice as soon as possible. The physician of choice would be an oncologist who is familiar with this disease.
If treated early, the odds of surviving to the five year mark are at least ten percent.  As is, most cases are not diagnosed until later stages, at which point the patient will unfortunately likely only survive for a matter of weeks or months. Early treatment options include surgery, chemotherapy and radiation.  Mesothelioma is a very aggressive cancer, but if is correctly diagnosed while still localized, surgery can be performed to remove the cancerous tissues.  In the late stages, after the disease has metastasized to other areas of the body, the focus is on keeping the patient as comfortable as possible.
Options include using surgical techniques to improve breathing and slowing progression with chemotherapy.  Palliative radiation, while not curative, can also be used to ease breathing as well as reduce pain and bleeding.  At present, there is promising research in finding a cure for mesothelioma, but the keys at present still lie in avoiding exposure and where this has already occured, in monitoring any unusual symptoms and seeking treatment immediately.

National Mesothelioma Awareness Day - September 26th

The Mesothelioma Research Foundation of America has a mission: fund research that will lead to the quickest cure for mesothelioma.
In 2001, this Research Foundation opened the Mesothelioma Laboratory under the supervision of Dr. Gill at the USC/Norris Comprehensive Cancer Center in Los Angeles. Since its opening and after two (2) million dollars in donation funding, we are extremely pleased by the results achieved and very excited about the possibilities of a more effective treatment over what presently is available for mesothelioma victims. The Veglin phase III clinical trials are soon to begin. But there is so much more research to be completed and comparatively little support from the Government specifically for treatments and a cure to mesothelioma. There just is not enough public awareness to get the Government's focus on this issue.
For example in 2009, the Department of Defense (DOD) expanded its mesothelioma cancer research program by awarding a few million dollars in funding to several research projects with a focus on the early detection of mesothelioma, identifying potential therapeutic targets, and an objective to launch a clinical trial for new therapy. But, the Government's funding for research on mesothelioma is much less than that of other cancers.
The National Cancer Institute (NCI) invested only 0.1 percent of its annual budget or less than $6 million on mesothelioma research from 2004 to 2007. By contrast, two comparisons put this budget number into perspective. Breast cancer research from the NCI alone received $685 million in 2009. And The Washington Post reports in a September 6, 2010 story that Defense Department spending on military music bands is estimated at more than $500 million a year. So it appears that it is more important to fund the blowing of brass instruments than it is to fund research for finding a cure to mesothelioma cancer that effects the lives of many who die from the desease they contracted from exposure to asbestos on the ships and in the shipyards of the US Navy.
These numbers highlight the urgent need to raise more money for mesothelioma research, which is the sole purpose of the Mesothelioma Research Foundation of America.

RESOLUTION for MESOTHELIOMA AWARENESS DAY

Whereas, mesothelioma cancer is an aggressive, asbestos related disease that affects the linings of the lungs, abdomen, heart or testicles;
Whereas, the heavy use of asbestos in manufacturing, industry and construction has been recognized as the worst occupational health disaster in U.S. history;
Whereas, asbestos was used in the construction of most office buildings, hospitals, public schools, power plants, automobile repair shops, refineries, and residential homes built before 1975;
Whereas, many mesothelioma cancer victims were exposed to asbestos on U.S. Navy ships and in shipyards;
Whereas, asbestos exposure is now believed to include many firefighters, police officers, and rescue workers including those from the 9/11/2001 Ground Zero terrorist attack;
Whereas, exposure to asbestos for as little as one month can result in mesothelioma fifteen (15) years later, or even longer;
Whereas, because of occupational, Navy service related, household, or incidental exposure to asbestos and the long latency of this cancer disease, tens of millions of Americans are now at risk for developing mesothelioma;
Whereas, for decades the need for research to develop effective treatments or cure for mesothelioma has been neglected or omitted;
Whereas, the outcome of neglecting research for a cure to mesothelioma limits today the benefit or effect of existing treatments and many patients with this disease die within twelve to fifteen months from diagnosis;
Whereas, in 2001, the Mesothelioma Research Foundation of America was formed to eradicate the life-ending and vicious effects of mesothelioma cancer with early progress in developing an effective treatment and potential cure which is found in Veglin (a non-chemotherapy drug solution) with research and clinical trials continuing to be made; and
Whereas, the establishment of Mesothelioma Awareness Day will help raise public awareness of the disease, how its latency can adversely affect the good health of many people, and for the immediate need to do research and discover an effective treatment or cure for this deadly cancer.
Now Therefore, be it resolved that September 26th be hereby declared Mesothelioma Awareness Day in [the name of your city or state].

Symptom Control in Mesothelioma

This section describes symptoms that may be experienced by people with Mesothelioma. It will include how these symptoms can be managed, based on medical evidence wherever possible. With care, much can be done to lessen the impact of most symptoms. In complex cases this requires the involvement of a variety of professionals - the multidisciplinary approach - such as doctors from palliative medicine and pain specialists, oncologists (cancer specialists), specialist nurses, physiotherapists, complementary therapists, psychologists and others. UK trade names for medicines are placed in brackets when used.
This section does not contain an exhaustive list of symptoms that may occur in mesothelioma. If you are concerned about any symptoms do go and discuss them with your GP, Macmillan nurse or hospital consultant and ask for specialist referral if the problems persist.

Legal Advice

Although the risks of asbestos exposure and its relation to Mesothelioma have been known since the late 1950s and 1960s* asbestos was still widely used in many industries until the late 1970s, and later to a lesser extent. Unfortunately, despite the apparent dangers some employers did not provide adequate protection or take measures to prevent exposure.
The following is a list of occupations known to have put workers at risk of asbestos exposure. The list is not complete and final and you may have worked in a different occupation and experienced exposure to asbestos.

Exposure was not limited to those who worked directly with asbestos. Mesothelioma can develop following para-occupational exposure. This means asbestos being taken outside the workplace and perhaps into the home on the work clothes of someone working with it, or following environmental exposure, for example, living near an asbestos factory.

The mesothelioma stages

 The stages of Mesothelioma are progressive. The Mesothelioma disease has four stages, the last of them being the worse and the most progressive.
     In stage I: the tumor respects the capsule, does not invade the surrounding structures, it is located, and has no accompanying adenopathy. At this stage the tumor is completely removable. Surgery is usually performed to remove the tumors that may affocated, and has no accompanying adenopathy. At this stage the tumor is completely removable. Sending mesothelioma in remission is still possible at that stage.
     In stage II: the tumor has the characteristics of stage I, but has intra-pleural adenopathy;
     In stage III: the local extension exists in the chest wall, in the pericardium or the diaphragm, and also implies the involvement of extra pleural lymph nodes;
     In Mesothelioma stages 2 and 3 the chances to send cancer into remission is virtually nil. Therefore, all treatments for these stages focus on improving the quality of life for the patient. Here the patient's comfort is of prime importance. Surgery does only provoke suffering for the patient, and it should be done everything it is necessary to alleviate the pain or discomfort.
     In stage IV: the tumor has distant metastases. If Mesothelioma has progressed to stage 4, the situation seems very bleak. At this stage, usually the cancer has spread beyond the region affected in other parts of the body, making treatment very difficult. Patients who have stage 4 Mesothelioma usually accuse high pain and discomfort. At this stage, all doctors can just try to manage the pain to make life somehow bearable for the patient. Surgery, chemotherapy and radiation are not usually part of the treatment at this stage of the disease.

Asbestos and mesothelioma


     Similar with the malignant Mesothelioma, the benign form is correlated with chronic exposure to asbestos and asbestos dust. Benign Mesothelioma manifests much more early, does not need an asymptomatic period so long as the malignant (20 - 50 years after exposure start). Because its nature is being benign, such a tumor can be treated very shortly after it has been discovered, its evolution being stopped promptly.
     There are cases of malignant degeneration of benign Mesothelioma, this being the main reason why treatment should be promptly established. This benign Mesothelioma may be a warning sign, causing the doctor to look for the existence of other diseases related to asbestos exposure, disease still asymptomatic but which may develop subclinical. Mesothelioma stage occurs in 70% of cases of asbestos inhalation, to subjects working in many industries.
     Benign Mesotheliomas are diagnosed more frequently in men compared to women, because the occupations which expose the patient to asbestos dust are: plumbers, plumbing, construction workers, locksmiths, welders. The first signs appear after about 35 years after exposure, which is sometimes for a very short period of time: pain in one side of the chest, pleurisy. There is no truly effective treatment of Mesothelioma, which gradually invades the chest wall and lungs.
     The incidence of the Mesothelioma disease is growing. Unfortunately, medicine can do nothing to prevent the development of these tumors in individuals who were exposed during the years 1950-1980. The important thing is that the diagnosis to be made as early as possible and treatment to be based on evidence given by clinical trials.

Mesothelioma the beginning


     One in 100 people born in 1940 will develop mesothelioma, which is almost exclusively a consequence of the exposure to asbestos. The primitive tumor of the pleura rarely appears until 25 years after the exposure, and especially it appears to over 50 years from the time of inhalation of asbestos fibers. Half of the current cases are in people over 70 years, of which 80% are men. For a man who had occupational exposure to asbestos during his youth, the risk of developing mesothelioma is of 1:5.
     Some researchers in Britain have identified two different patterns of occupational exposure: men born in 1920 who were employed in building, which have had a progressive level of exposure to asbestos the whole lifetime, while the men born around the 1950s had a higher exposure (greater than the previous generations), of short duration, in the early career, which ended up at the age of 30 years, when the use of asbestos was prohibited.
     Asbestos is an excellent building material and it has been widely used in construction, especially in the interwar and postwar period, until 1980. Although it was banned in countries like the U.S. or UK, asbestos is still used in some places in motor vehicles, electrical products (hair dryers, air conditioning), the insulator, in textiles and even cosmetics. In developing countries, the import of asbestos still remains at high levels.
     In Europe, the estimated average death rate for the period 2015-2020 is of 2000 persons a year, with a higher rate for Eastern European countries. The incidence of mesothelioma is already in decline in the U.S., the measures to limit the imports and to ban the asbestos in the building being taken early. Many countries, however, will face an epidemic of mesothelioma in the next decade, following a belated awareness of the harmful effects of asbestos.

What mesothelioma is


     Mesothelioma is a slow-growing tumor and it originates from the mesothelial cells structure: parietal pleura, peritoneum or tunic vaginalis of the testicle (not reported cases of pericardial mesothelioma). But the most common location is the pleura. The mesothelioma symptoms consist of chest pain or dyspnea (difficulty in breathing) that results from the accumulation of fluid in the pleural cavity. Sometimes the mesothelioma disease can be asymptomatic, installed insidious (as the patient's neglect) or even symptoms free.
     Once identified the pleural disease, the differential diagnosis must be made with pleural plaques and other causes of pleural effusion (a special issue represents a break of a secondary malignant pleural adenocarcinoma). The cytology examination of pleural fluid and the fine needle biopsy are often inconclusive, as it is necessary to identify the collection of some appropriate pieces in order to identify mesothelioma. Survival from diagnosis is 1 year on average, with variations large enough, given the slow growth pattern. The effectiveness of various methods of treatment is reduced.

Mesothelioma treatment


     The mesothelioma treatment consists in removing the tumor. Benign mesothelioma can be treated relatively easily, unlike the malignant mesothelioma, which has a high mortality and benefits only of symptomatic and palliative treatment. The node removal is done surgically, the patients' prognosis being very good. If there is pleural effusion, the patients will be fit a drain tube to evacuate the liquid. In the presence of a prompt treatment, the prognosis of patients is very good. There is the possibility that in 1 of 10 patients, that the node to recurrence (the mesothelioma disease to have a recurrent character). In some cases the recurrence manifest even at 10 years after the removal of the first node string.
     All the patients operated by benign mesothelioma must perform regular radiographic checks to monitor the progress. The emergence of mesothelioma may be a sign of the existence of some disseminated disease due to exposure to asbestos. Asbestos mesothelioma is deadly. Therefore it is recommended that patients to be investigated in detail in the early diagnosis of other diseases with the same origin.
     The survival rate is, during treatment (very wide) between 16 to 18 months. Very few patients survive up to 5 years, even during mesothelioma treatment. The prognosis is reserved in all cases and depends heavily on:
  • Histological type of cancer;
  • The stage at which treatment begins (if it is greater than II, survival is much reduced);
  • Therapeutic possibilities;
  • The intensity of the felt chest pain;
  • The level of hemoglobin (the more reduced, the more the prognosis is worse);
  • Leukocytosis;
  • The LDH level greater than 500 IU / L;
  • The patient age at diagnosis;
  • Sex male.

Diagnosis of mesothelioma


     The diagnosis of malignant mesothelioma may be determined by:
  • Highlight the break effusion;
  • The collection of pleural biopsy tissue fragment by thoracoscopy (it can establish a diagnosis of certainty in over 98% of cases).

Mesothelioma


Mesothelioma is a rare form of cancer that attacks the mesothelium, a membrane that surrounds and protects many of the body’s internal organs as well as the walls of the chest and abdominal cavities. The term almost always refers to malignant mesothelioma, as benign cases of the disease are a small fraction of the overall occurrences.
Mesothelioma has become a widely known and publicized disease despite its relative infrequency because of the fact that the only proven cause for its development is asbestos exposure – specifically, inhalation or ingestion of tiny, microscopic asbestos fibers that eventually cause the development of mesothelioma. Asbestos related diseases are among the primary industrial medical afflictions of the nineteenth and twentieth centuries.
The disease has an unusually long period of latency. Persons exposed to asbestos may not develop symptoms of asbestos related disease for twenty to fifty years after the asbestos exposure has taken place. For that reason, the widespread use of asbestos through the 1970s is still causing new diagnoses of mesothelioma today among workers who have long been retired. Although today there are only about 3,000 new cases of the disease diagnosed in the U.S. annually, mesothelioma has taken tens of thousands of lives in this country since asbestos was introduced as an industrial material in the late 19th century.
